When you voluntarily complete the Confidence in Every Step™ questionnaire, we may also receive information about balance, mobility, fall history, physical activity, injuries, recovery, goals, emergency contacts, and other information you choose to provide.
Please do not submit information that is not reasonably necessary for scheduling or fitness/wellness screening. JB Per4mance does not use the questionnaire to diagnose or treat medical conditions.

- Schedule and administer appointments and programs.
- Prepare for fitness/wellness screenings and tailor coaching.
- Send confirmations, reminders, service messages, and requested communications.
- Process payments and maintain business records.

We may use service providers that support hosting, email, scheduling, forms, payments, analytics, security, and website operation. These may include Hostinger, WordPress, Amelia, Google Calendar, and Stripe when their respective services are enabled. Each provider handles information under its own terms and privacy practices. Payment card information is processed by the selected payment provider and is not intended to be stored directly by JB Per4mance on this website.
